Diabetic cardiomyopathy (DCM), a major risk factor contributing to diabetes-related morbidity and mortality, manifests as a pathophysiologic condition in humans independently of coronary artery disease or hypertension. Several transcription factors intricately participate in the initiation and progression of associated processes. Among these, nuclear factor kappa B (NF-κB), a pleiotropic transcription factor, assumes a crucial role in regulating oxidative stress, inflammatory response, endothelial dysfunction, myocardial fibrosis and hypertrophy, and cardiomyocyte apoptosis in DCM. However, the precise role of NF-κB in DCM remains a subject of ongoing investigation. This article provides a comprehensive review of NF-κB's involvement in several processes associated with DCM.
