Secondary Acute Promyelocytic Leukemia after Treatment of Early Breast Cancer: Report of Three Cases and Review of the Literature

The consequence of successful treatment of cancer has been a parallel increase in the long term complications in the expanding population of survivors. Breast cancer is the most common female cancer and second cause of cancer related mortality in the western world. Increased survival rates due to early detection by mammographic screening and improved therapeutic approaches have been counteracted by late side effects particularly secondary acute myeloid leukemia (AML). Therapy related AML (t-AML) occurs in 0,3 – 1,7% of breast cancer patients receiving antineoplastic chemotherapy. t-AML and myelodysplasia (t-MDS) have relatively poor prognosis compared to de novo counterparts. Acute promyelocytic leukemia (APL) is a unique AML subtype with t(15:17) and accounts for 5-8% of all AML cases. The incidence of therapy related APL (t-APL) has increased in the last decade and it accounts for 22% of APL cases versus the 5% in the earlier reports. Here were report 3 patients with t-APL which occurred following adjuvant breast cancer treatment.
