Delayed diagnosis of IPA resulted in the development of a pneumothorax, a rare consequence of fungal pneumonia.</span><span style="font-size: 11.0pt; font-family: Cambria; mso-fareast-font-family: ";Times New Roman";; mso-bidi-font-family: ";Times New Roman";; mso-ansi-language: EN-US; mso-fareast-language: EN-US; mso-bidi-language: AR-SA; mso-no-proof: yes;"> Respiratory virus infections are often harbingers of other infective organisms in HSCT recipients, and more aggressive diagnostic investigations such as computed tomography scans of the thorax and bronchoscopy with bronchoalveolar lavage should be considered early in <span style="mso-bidi-font-weight: bold;">any HCT patient presenting with respiratory virus pneumonia, particularly if atypical features are present or delayed recovery occurs.</span></span><!--EndFragment-->
