High-dose teicoplanin combined with daptomycin cured infective endocarditis and septic pulmonary embolism caused by methicillin-resistant Staphylococcus aureus: A case report and literature review

Septic pulmonary embolism (SPE) secondary to infective endocarditis (IE) caused by methicillin-resistant Staphylococcus aureus (MRSA) is rare in extensive burn cases. MRSA is a highly pathogenic bacteria and IE is very difficult to treat. The best therapy for serious MRSA infection remains controversial. Herein, we reported a patient with SPE secondary to IE caused by MRSA, who was treated with teicoplanin (1000 mg twice daily as the loading dose and 1000 mg daily as the maintenance dose, based on the severity and body weight 95 kg, 12 mg/kg) and daptomycin 500 mg daily. The patient was successfully cured with slight liver function impairment. So high-dose (>800 mg) teicoplanin is a potential option to treat serious SPE and IE caused by MRSA.
