Pituitary metastasis in HER-2 positive breast cancer with initial presentation of hypernatremia: Two case reports

Pituitary metastasis is rare and typically has a poor prognosis. It is usually asymptomatic and is predominantly found in elderly patients with a history of advanced-stage primary cancer with multiple distant metastases. Unfortunately, there is currently no valid clinical diagnostic criterion for pituitary metastasis, nor is there a standard treatment plan. In this article, we report two cases of pituitary metastasis resulting from breast cancer. The patients, aged 50 and 63 years, presented with specific manifestations of pituitary metastasis, and both underwent surgical biopsy for tissue-based diagnosis. After the biopsy, patient 1 received palliative radiotherapy, capecitabine, and lapatinib; patient 2 received paclitaxel, pertuzumab, and trastuzumab. Both patients received hormone replacement therapy for panhypopituitarism. Patient 1 developed new brain metastases six months after capecitabine and lapatinib treatment. She then received local palliative radiotherapy, targeted therapy with bevacizumab, and chemotherapy with cisplatin and etoposide. She is currently in good clinical condition without evidence of new metastases while patient 2 was lost to follow-up afterward.
