Successful use of intra-aortic balloon pump in sepsis-induced cardiomyopathy: A case report

A 78-year-old female presented to our emergency department with worsening breathlessness and high-grade fever over two days, and was found to be in profound septic shock. She required immediate intubation, mechanical ventilation, fluid resuscitation, vasopressor support, antibiotics and intensive care unit admission. Over the next 2 days, she developed worsening shock, sepsis-induced cardiomyopathy and multi-organ failure, leading to an episode of pulseless electrical activity. An intra-aortic balloon pump was inserted within an hour of cardiopulmonary collapse. The patient's condition improved dramatically. She was weaned off intra-aortic balloon pump (IABP) and inotropes by the seventh day, and eventually discharged well and ambulant. Thus, in our patient, the use of IABP enabled a dramatic recovery in the setting of severe septic cardiomyopathy with multi-organ dysfunction by improving hemodynamics and supporting myocardial function while allowing time for sepsis treatment to take effect. We recommend conducting larger studies to further study this phenomenon.
