Crucial Conversations and Conflict Resolution Interprofessional Simulation

Abstract Introduction Communication is considered a core aspect of interprofessional collaborative practice. Developing basic communication skills is a common area for health professions education, but health professions students often have little knowledge about or experience with interprofessional communication. Professional hierarchies created by demographic and professional differences are common but create dysfunctional communication patterns working against effective interprofessional teamwork. Learning to have a crucial conversation or resolve a professional conflict with confidence helps health professionals improve team-based care and ensure patient safety. Methods This is a simulation/role-play activity for interprofessional learners to acquire knowledge about crucial conversations and conflict resolution and to demonstrate their skills with interprofessional team members. This learning activity was developed for learners on an interprofessional family medicine clerkship as an educational adjunct to be used during protected curricular time. It was developed and piloted during the summer of 2013 and has been integrated as a component of the clerkship curriculum since October 2013. This activity has been completed by students on four clerkships and from three different professions (medicine, nursing, and pharmacy). During each clerkship, 12 students participate in the activity, four from each profession. Results A total of 48 students participated between October 2013 and September 2014. We have pre− and postactivity survey results providing evidence that learners achieved the educational objectives. Student confidence in conflict resolution increased, and students saw value in the specific communication tools highlighted in the simulation. A common theme that emerged as a lesson learned was the raised awareness of professional hierarchies. Discussion Overall, this learning activity was a great exercise for intentional interprofessional education and reflection to be built into the daily interprofessional clinical experience. In addition, this activity was a great way to have a crucial conversation about crucial conversations and conflict within health care teams.
