Background: Diabetic ketoacidosis (DKA) is an acute complication of diabetes mellitus, which is diagnosed by the triad of hyperglycemia, metabolic acidosis, and ketosis. This entity can further lead to various complications, but rhabdomyolysis is rare. Patient's Concern: A 34-year-old male presented with generalized muscle weakness and paresis following acute management of DKA. Laboratory evaluation confirmed elevated muscle enzymes, myoglobinuria, and acute kidney injury, consisting of rhabdomyolysis attributed to severe hypophosphatemia. Intervention: He was managed conservatively in the form of electrolyte correction, which led to a favorable recovery. Lesson: Hypophosphatemia, leading to rhabdomyolysis in DKA, is described scarcely in the current literature, and knowledge of this can easily prevent unnecessary morbidity and mortality.
