We describe our initial experience with four consecutive cases of transcatheter aortic valve implantation (TAVI) in bicuspid aortic valve (BAV) anatomy with an annular perimeter in range of 100 mm which is beyond the range of any of the recommended sizes as per instructions for use (IFU) for an appropriate size valve from any of the commercially existing makes of transcatheter valves. The techniques used and principles applied for doing a successful transcatheter procedure in these cases are presented.
