Comparison of Effectiveness of Ultrasound Guided Injection and Injection with Palpation to the Radiocarpal Joint

INTRODUCTION: In this study, we aimed to compare the ultrasound guided injection and injection with palpation using MR arthrography regarding the differences in success rate and the extravasation rate of the two techniques. METHODS: In this prospective study, we planned to include patients to whom an injection to the wrist was performed due to an indication. First, forty patients were divided into two groups and, ultrasound guided injection was performed to one group whereas injection was performed with palpation to the other. After the procedures, MR arthrography was performed to assess the reach of the contrast in to the joint space as well as extravasation into the adjacent soft tissues. RESULTS: There was contrast in the joint space in all injections guided by ultrasound (100%). Seventeen out of 20 injections which performed with palpation, there was contrast within the joint space (85%). Extravasations were seen in 3 patients (15%) with ultrasound guided injections group and 5 patients (25%) with palpation group. DISCUSSION AND CONCLUSION: Injection to the wrist either under ultrasound guidance or with palpation, there was no significant difference by means of success and extravasation rate. Injection with palpation is a rapid and cost-effective technique however its success rate is hindered by pathologies that affect the palpation of the joint structure. In such circumstances, ultrasound guidance increase the success rate of the procedure.
